Who we support

Total Care provides a full range of domiciliary care and support services to assist those with both basic and more complex needs. You can choose a package of care tailored to your individual needs, to assist you in living safely and independently in your own home.
 
Each service is led by highly experienced managers and each care team member is specifically recruited according to previous care experiences. 

To ensure the diverse, changing care needs of our clients are consistently met and understood, care team members receive regular supervision and continuous professional development training.

Elderly Care at Home

Total Care elderly care at home service offers the highest quality personal care and support at home. Our aim is to empower our clients with the option of remaining in their own homes with dignity, independence and control over their lives. Regardless of the level of support needed, a member of the Total Care team is available 24 hours a day, 365 days a year.

For some, the reassurance and companionship gained from having one of our carers visit for a few hours a day to assist around the house will be enough. Others however will require more intensive 24 hour care which can be provided on a bespoke basis. 

Regardless of the level of need the team at Total Care will always work with you to design a service package that will enable you or your loved one to live life at home, to the fullest.

Some examples of the types of services that Total Care team can assist with are detailed below however, understanding that no two people are the same and everyone’s’ situation is different, please do not see the list below as restrictive - we always design our services around your unique circumstances.

End of Life Care

WE'RE HEre TO provide 24 hour support and care

At Total Care we know that, given a choice, many individuals would prefer to die at home, however for both them and their families the fear, emotional turmoil and practicalities of caring for someone at this stage of life can be challenging. 

Reflecting this, we have a dedicated palliative care service to support clients and their loved ones at home. The palliative care team at Total Care recognizes that, due to medical advances, people with serious illnesses are often living in poorly conditions for longer. Ensuring dignity, compassion and respect in these circumstances is a priority in the care we provide.

Our ethos at Total Care is to provide the highest quality care and support possible. Our service has been designed to complement other palliative care teams and hospices often involved in the person’s end-of-life care. 

Support from 24 hour care enables freedom of choice and flexibility. The Total Care team work in partnership with community palliative care teams, other specialists from the multidisciplinary team as well as family and friends.

With over a decade of experience we aspire to add life to days and enable clients and their families to fulfill their choices. As part of the training our team members learn and discuss differing cultural needs and practices of clients at end of life.

Palliative Care

Palliative care is part of our supportive care plan

Palliative care is the active holistic care of patients with advanced progressive illness. Management of pain and other symptoms and provision of psychological, social and spiritual support is paramount. The goal of palliative care is achievement of the best quality of life for patients and their families.
Palliative care aims to:
Affirm life and regard dying as a normal process
Provide relief from pain and other distressing symptoms
Integrate the psychological and spiritual aspects of patient care
Offer a support system to help patients live as actively as possible until death
Offer a support system to help the family cope during the patient’s illness and in their own bereavement
Reablement services are for people with disabilities and those who are frail or recovering from an illness or injury. The aim is to help people regain the ability to perform their usual activities, like cooking meals, washing and getting about, so they can stay independent and live in their own home. Reablement services are provided by local authorities, sometimes in partnership with the health service.

If a person is referred to a reablement service, reablement workers will visit them in their home, assess their abilities and needs and agree goals. Over the next few days or weeks the individual will be supported to regain physical function, relearn skills and if necessary learn different ways of doing daily tasks such as meal preparation, washing and dressing.

Reablement

Total Care reablement support is a short and intensive service, delivered in the client’s home

This service is offered to people with disabilities and those who are frail or recovering from an illness or injury. The purpose of reablement is to help people who have experienced deterioration in their health and/or have increased support needs to relearn the skills required to keep them safe and independent at home.

People using reablement experience greater improvements in physical functioning and improved quality of life compared with using standard home care. It is common for people to be unsure about what reablement is, and for family and friends to be uncertain about what it means for them. This “at a glance” explanation is designed to help our clients understand what reablement is, and what family and friends can do to help.

The focus of reablement is on restoring independent functioning rather than resolving health care issues. The objective is to help people relearn how to do things for themselves rather than the conventional home care approach of doing things for people. 

People using reablement experience greater improvements in physical functioning, health-related quality of life and social care outcomes compared with people using standard home care.
We access training from many organisations, our healthcare assistant team are able to support clients with PEG feeding, hoist mobility, personal care, appointments and family visits etc.

One of our healthcare seniors will meet with you, your family, social worker(s), OT and any other people you wish to be involved in designing your package of care. Your home care package provides a coordinated package of services tailored to meet your specific care needs.

The home care can be funded privately or you may be entitled to social care funding if you have a social worker.
A home care package provides services that can:
Help you to stay at home for as long as possible
Give you choice and flexibility in the way that care and support is provided to you

Bespoke 24 hour Care Packages

If you want to stay in your own home but need some help to achieve this, a home care package may be for you. We provide services to people with life-limiting illnesses. We encourage, support and maximise people’s enjoyment of life by providing the service that people want.

We always aim to provide individualised care packages based on getting to know our clients and what they hope to achieve from their support team, their dreams and their aspirations.

What types of services are provided?

The types of services provided under a Home Care Package will depend on your needs. We will co-ordinate care and services to support you at home. A range of services may be provided under a Home Care Package, including:

  • Personal care – such as help with showering or bathing, dressing, mobility, meal preparation and eating
  • Support services – such as help with washing and ironing, house cleaning, basic home maintenance and transport to help you do shopping, visit your doctor or hospital appointments
  • Feeding – prepare, cook and assist you to eat it your meals, PEG feed support, grocery shopping, nutritional support or just reminding you to have a meal.

We can also negotiate other services to support you to live at home where these are identified in your care plan. For example, you may benefit from some aids and equipment to assist with mobility, communication or personal safety. Whatever is agreed must be affordable within the budget that is agreed at the beginning of your plan, although if you are paying privately, you can change the budget to suit your financial availability.

How do I know if a Home Care Package is right for me?

Ask yourself these questions:

  • Are you struggling to remain independent in your own home?
  • Are you having difficulties with some aspects of living at home, but you’re still able to manage most of the time?
  • Do you think if you don’t get some help at home, you might have to go into supported living or residential care home before you’re ready?

If you answer ‘yes’ to any of these questions, then you may be eligible for a continuing health care package and you should speak to a Social Worker or Occupational Therapist. If you have savings or a regular income you may wish to fund a Bespoke Home Care Package.

There is not a minimum age for accessing a Home Care Package with us, but you will need to get confirmation from the NHS or Social Care funding if you are unable to pay for your own package.

What types of packages are available?

There are four levels of Home Care Packages which are designed to give you the care you need:

Level 1 supports people with basic care needs i.e. washing, cleaning, mobility etc.

Level 2 supports people with low-level care needs i.e. meds prompt, routine check.

Level 3 supports people with intermediate care needs i.e. personal care, dressing, meals.

Level 4 supports people with high-level care needs i.e. hoist, PEG feed, health watch.

What is a Home Care Agreement?

All people receiving care under a Home Care Package must be offered a Home Care Agreement. This is an agreement between you and your home care provider that sets out what care and services you will receive.

How do I get a Home Care Package?

You will first need to consider the costs or funding for your package. Contact your GP, Social Worker or OT to find out if you are eligible for funding, if not you may have to fund the package out of savings or your family may wish to pay for your package of care.

If you are assessed as eligible for a package, you will be able to get Direct Payment where you can pay for your care yourself or you may wish to get your finances dealt with by a not for profit organisation i.e. Compass Disability, A4e or Enham.

Once you have your finances agreed, a package of home care can be devised to meet all your needs and ensure that costs stay within the set budget.

EXAMPLE: you may wish to receive support in the morning to get out of bed, showered, dressed, breakfast and ready to start your day. You may book an hour or two to visit the Dr, hospital or a friend for lunch. You may wish to book a visit from a podiatrist, nail technician or hairdresser. You may require a visit at tea time to prepare you an evening meal that was ordered and delivered by a health food company. Bedtime may require a bath or shower, prepare for the night and quickly tidy around and lock up.

Will my current care package be affected?

If you currently receive care from another provider, you are entitled to choose who you wish to provide your care/support package. A change of provider does not need to be a stressful experience. You will need to agree your Home Care Package before you give notice to your current provider to ensure a seamless transition occurs and all your needs are met. If you choose to have a Home Care package with us, we can help you to find out your notice period and assist you in informing them of your wishes. We can work closely with you and your Social Worker, family members or anyone else that has your best interests at heart and understands your finances.

Waking Night Care

At Total Care we are able to provide waking night cover. Our carers are on-call at all times during the night and are awake, alert and making certain our client is sleeping, resting or is at least safe, sound and secure. Our shifts are from 10pm through to 7am.

Respite Care

Total Care understands that being a full-time carer is a very demanding role. Respite care breaks are an opportunity for carers to take a holiday or simply spend some time looking after their own needs, safe in the knowledge that their loved one is receiving the very best care. And respite care can be a valuable break for older people too – it’s a chance to meet new people, enjoy a change of scenery and try out some new activities and experiences.

We tailor our respite stays to each person’s individual needs and our care teams have specialist expertise in supporting older people who have complex needs, physical disabilities or restricted mobility, and conditions such as Parkinson’s disease and Multiple Sclerosis. Respite care can last for just a few days or as long as is needed.

Each client has a personal care plan that takes into account their support needs, interests, likes, dislikes, and daily routines.
